We at Lane Ends Surgery take patient safety very seriously. We follow the latest advances in medical research and continually update and review our clinical practice to ensure patient care is of the highest standard and you are receiving the most effective treatment.
If you are taking any of the following medications:
· WARFARIN *
· AZATHIOPRINE *
· METHOTREXATE *
· TACROLIMUS (ADAPORT) *
· MYCOPHENOLATE MOFETIL (MMF - CELLCEPT) *
· LITHIUM (PRIADEL) *
· DIAZEPAM / TEMAZEPAM / LORAZEPAM / ZOPICLONE / ZOLPIDEM (CD]
· GABAPENTIN / PREGABALIN |CD|
· TRAMADOL / FENTANYL OR BUPRENORPHINE PATCHES |CD|
· MORPHINE / OXYCODONE (CD]
As these medicines can potentially cause serious side effects, they need regular monitoring and some may be Controlled Drugs [CD] whose use is governed by law because of the risk of drug dependence or abuse. For these reasons, the prescribing and supply of these medicines will be monitored to ensure patients do not suffer any harm as a result of their use.
Medication requiring monitoring will:
· NOT be placed on your repeat prescription. The medicines will not be visible on paper copies of your repeat prescription and will not appear online in the list of repeat medicines available to order. It will also not be possible to obtain these medicines from your Pharmacy using repeat or "batch" dispensing.
· You will need to complete monthly requests for these medicines. This can be done using paper request forms available at Reception, through prescription line or via the NHS app by adding a message.
· A Clinician or a member of the pharmacy team may need to contact you before issuing your prescription. It is therefore essential that you provide the surgery with up to date contact details including your mobile phone number.
· If regular bloods tests are needed (indicated by a * against the medication listed above ) to monitor the effects of your medicines, a Clinician will not be able to issue your medication until the results of these tests are available to the practice. Please have any blood tests done promptly before you are due to run out of your medication. You can book blood tests via the NHS App.
· Please remember to order your medicines in good time. As additional checks may need to be carried out, it may take longer to provide prescriptions.
In addition to the above medication, each year we invite a large percentage of our patients for an annual chronic illness review.

The purpose of an annual review is to ensure patients are taking the right medication and receiving the best possible care for their condition(s).
